A 63yr old male came with chief complaints of bilateral knee pain and knee stiffness since 2 to 3 yrs 

Aggrevated  since 2 to 3 months 

History of present illness: 

Patient was apparently a symptomatic to 3 years back then he developed bilateral knee pains insidious in onset, gradually progressive to the current stage . joint stiffness present,  can’t get up from sitting position decreased range of movements difficulty in extension of knee tingling sensation of both upper and lower limbs present ,back pain on and off Radiating from Lower back to both hips, swelling not present, No redness, no history of trauma , Pedal Eddie Maher present below knee on prolonged sitting. No history of decreased urine output, no burning micturition , sob present grade 2 , no cough , no palpitations, no chest pain , no abdominal pain , no constipation, no loose stools , no fever 

Past history : 

N/k/c/o DM,HTN, CVA, CAD , TB , epilepsy , asthma 


Personal history : 

Occupation : farmer 

Diet: mixed
Appetite : normal
Bowel: regular
Bladder: regular
Sleep: adequate
Habit : alcohol regular 90ml alternate days since 38yrs 
Tobacco smoke chutta 20- 30 for 3 yrs per day stopped 1yr back  

Family H/O not significant


General examination
Patient is Conscious, coherent, cooperative, moderate built and moderately nourished
No pallor, icterus, cyanosis, clubbing, lymphadenopathy,

SpO2: 98 @ RA


Systemic examination 
CVS:S1S2 hears, no murmurs
RS:BAE+ , NVBS +
Dyspnoea grade 1 
P/A: soft, non Tender
CNS: NAD

Bilateral osteoarthritis of knee joint with grade 1 pristine valu with right mild hydroureteronephrosis
